Forms 

• Social Event Form — This form is used by all Greek organizations to register ALL social events (parties, mixers, formals, takeovers, etc.)
• Failure to Register a Social Event Policy
• Community Service Form — This form is for chapters that need to report or submit community service hours.  Information is entered directly into the online form. 
• Grade Consent Form— This form needs to be completed or signed by any new members or incoming transfers for EACH semester. Once an individual signs the consent form, it is valid for the remainder of his/her academic career.
• Roster Update Form — This form is for chapter presidents to make updates to their chapter roster and should be submitted at the beginning and end of each semester.
• Incident Report Form — Primarily for the Interfraternity Council but accepted for all councils, this form should be used to report any incidents as they relate to Greek Life, University of North Texas, Council, and/or individual chapter policy(s).

Greek Week

Greek Week 2012 Packet - The Greek Week packet details all requirements and expectations associated with Greek Week, as well as a full schedule of information.

Greek Housing Forms & Policies 

• Greek Life Housing Policies – This is the UNT Office of Greek Life policies/procedures for Greek housing.  Included in this docuement is Missing Resident Student(s) Procedure, Fire, Safety, and Severe Weather information, Policies and Procedures for Emergency Contact Information, as well as several tips/suggestions for fire, safety, housing security, renters insurance, etc.
• Emergency Maps and Evacuation for Greek Housing - Please review UNT's Emergency Procedures and evacuation maps for fraternity/sorority housing
• Emergency Contact Form – This form is for residents of Greek Housing (Fraternity OR Sorority Row) and must be completed at the beginning of each semester.
• Missing Person Contact Form - This form is for residents of Greek Housing and must be completed at the beginning of each semester.
• Greek Housing Missing Student Report – In the event a of missing resident, this form should be filled out by the reporting student(s)/advisor/house manager.

Academic Policy & Grade Reports 

Greek Academic/Good Standing Policy - This policy indicates minimum requirements for chapters to remain in good standing as well as sanctions for groups who do not meet the requirements.

Standards of Excellence

The Standards of Excellence is a program used to promote a high quality level of performance among all Fraternities and Sororities on campus. Due at the end of each Fall semester, the Standards of Excellence are to be completed by every Greek-letter organization at UNT.

• Order of Omega – This application is for any member of a Greek-letter organization who wish to join the Greek Honorary Society, Order of Omega (eligibility requirements lised in document). 
• Greek Ambassador – The Greek Ambassador program helps educate the UNT a local community, while highlighting the UNT Greek community with a select group of fraternity and sorority members (eligibility requirements listed in document). 
• Rho Gamma - For all Panhellenic women seeking to become a recruitment counselor. DUE February 14th to the Office of Greek Life.
